What Recovery Looks Like After ACL Reconstruction

Knee surgery may repair the ACL, but recovery is where you rebuild strength and stability. Each milestone matters, from managing swelling in the first week to finally being able to walk confidently again.

The First Few Days

In the days immediately following your ACL surgery, you’ll probably notice swelling, bruising, and stiffness around your knee. You will wear a brace and use crutches to keep weight off the joint. 

To manage pain and inflammation, you’ll be encouraged to rest, elevate your leg, and use ice regularly. Your care team may also start you on light physical therapy within the first week to help with circulation and prevent too much stiffness, but your movement will be limited at this stage.

Recovery Milestones

Recovery from ACL reconstruction happens gradually. Timelines can vary depending on age, injury severity, and your body’s healing process, so the following should be viewed as general guidance:

  • Weeks 1–2: Your main goals are managing pain and swelling and slowly beginning to move the knee. We may have you start light exercises to regain a small range of motion.
  • Weeks 3–6: You’ll likely begin putting more weight on your knee and working on basic mobility. Your physical therapist will guide you through strengthening exercises during this stage.
  • Weeks 6–12: As your strength and flexibility improve, you may be able to walk without crutches. We will monitor your progress and adjust your rehab plan as needed.
  • Months 3–6: You’ll focus on more functional exercises to rebuild balance, coordination, and control. Some low-impact activities may be added, depending on how you’re doing.
  • Months 6–9+: If you’re an athlete, this is when we may begin testing your readiness for sport-specific movements and developing a plan for returning safely.
